Pre-Participation Physical Examination Form

In order to participate in athletics at Oratory Prep, a current and valid pre-participation physical examination packet must be on file at the school. Please note that this is required for all students regardless of whether or not they plan to participate in an athletic program. Physicals are valid for 365 days from the date of exam written on the packet. For a physical to be valid, it must be completed and signed by the parent/guardian of the student-athlete, student-athlete and physician in the designated areas within the form. It is STRONGLY recommended that you maintain a copy of the physical for your records.

This form is accessible as part of the checklist of required forms as referenced in the “Required Main Office and Health Forms” email that went out to all families in May. The checklist can be accessed in your MyOPS portal (https://oratoryprep.myschoolapp.com). It is listed as “NJ-DOE Preparticipation Physical Evaluation Form”. That step contains a link to download the form.

The Health History Page (page 1 of the packet) is to be completed by the parent/guardian. All “Yes” answers must be explained in the designated area. This page also requires a signature from both the parent/guardian and the student-athlete. The Physical Examination Form (page 3) and Clearance Form (page 4) need to be completed entirely by the physician who performs the physical examination.

After the packet is completed, please scan and upload a copy of all pages as one file to the step in your checklist. No other physical examination paperwork will be accepted.

Arbiter Athletic Registration

Athletes at Oratory Prep must be registered using the Arbiter System. Arbiter is a secure platform that allows us to keep accurate records of all state required sign offs, emergency contact information and create accurate rosters for each season.  

Information Needed to Register

It will be helpful to have the following information handy to allow for accurate completion of your online registration.
  • Sport your son plans on participating in
  • Contact information for 2 emergency contacts
  • Your son’s physician’s name and information
  • Your insurance information
  • Name and dosage of medication your son may take
 

Registration Process

A parent/guardian should contact Pat Gist, Athletic Director, to begin the registration process.
