Pre-Participation Physical Examination Form

In order to participate in athletics at Oratory Prep, a current and valid pre-participation physical examination packet must be on file at the school. Please note that this is required for all students regardless of whether or not they plan to participate in an athletic program. Physicals are valid for 365 days from the date of exam written on the packet. For a physical to be valid, it must be completed and signed by the parent/guardian of the student-athlete, student-athlete and physician in the designated areas within the form. It is STRONGLY recommended that you maintain a copy of the physical for your records.

This form is accessible as part of the checklist of required forms as referenced in the “Required Main Office and Health Forms” email that went out to all families in May. The checklist can be accessed in your MyOPS portal (https://oratoryprep.myschoolapp.com). It is listed as “NJ-DOE Preparticipation Physical Evaluation Form”. That step contains a link to download the form.

The Health History Page (page 1 of the packet) is to be completed by the parent/guardian. All “Yes” answers must be explained in the designated area. This page also requires a signature from both the parent/guardian and the student-athlete. The Physical Examination Form (page 3) and Clearance Form (page 4) need to be completed entirely by the physician who performs the physical examination.

After the packet is completed, please scan and upload a copy of all pages as one file to the step in your checklist. No other physical examination paperwork will be accepted.

RSchool Athletic Registration

We use a system called rSchool for athletic registrations. The rSchool system is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. When you register through rSchool, the system keeps track of your information in your rSchool profile and will sync with the other programs we use in the athletics department to seamlessly transfer data. We open registration 60 days prior to the start date of each season. If your son participates in all 3 seasons, although you must complete this process 3 times, most of your information will be saved. Registration for the fall high school season will open on June 14, 2021.

The rSchool registration is where you will complete all of your emergency information and sign off on all of the information that the state athletic association, NJSIAA, requires. These will include information on steroids, opioids, and eye injuries. Please complete all of these and the health history questions. When your registration is complete, you will see a  page that you can print for your records.
